WAIVER AND RELEASE OF LIABILITY
By checking the box, I acknowledge and agree to the following:
1. Assumption of Risk & Release of Liability
I understand that participation in a Volley Monster event (hereinafter the “Activity”) carries inherent risks, including but not limited to serious injury, death, or property damage. I voluntarily choose to participate and assume all associated risks.
I hereby release and forever discharge Volley Monster, LLC, located at 852 Maple Rd, Gambrills, MD 21054, along with its affiliates, managers, members, agents, attorneys, staff, volunteers, heirs, representatives, predecessors, successors, and assigns (collectively “Releasees”), from any claims, liabilities, or damages arising from my participation in the Activity.
2. Indemnification & Responsibility
I agree to indemnify, defend, and hold harmless the Releasees against any claims, suits, or actions of any kind brought by me or anyone on my behalf, including legal fees and related costs.
I acknowledge that Volley Monster, LLC is not responsible for errors, omissions, or actions by any third party conducting activities on its behalf.
3. Medical Authorization & Financial Responsibility
In the event of a medical emergency, I authorize Volley Monster, LLC to provide necessary medical care, including but not limited to first aid, CPR, AED use, and emergency transport. I assume full responsibility for any medical costs incurred and understand that I should carry my own health insurance.
4. Participation Requirements
I certify that I am physically fit and have trained appropriately for this Activity. I will abide by any decisions made by Volley Monster, LLC officials regarding my eligibility to participate.
5. Parent/Guardian Acknowledgment (For Participants Under 18 Years of Age)
If the participant is under 18 years of age, I, the undersigned parent or legal guardian, acknowledge and accept all risks on behalf of my child. By checking the box below, I agree to the terms of this waiver and release.
